YEAR 1 | PAGE 18C - STANDARDIZED PATIENTS, CHECKLISTS, DOCUMENTATION +

C. WHAT SCHOOL CLINICAL-SKILLS ASSESSMENTS COMMONLY REQUIRE — History taking

Establish rapport, identify agenda, characterize symptoms, obtain relevant background/social context, summarize and close.

C. WHAT SCHOOL CLINICAL-SKILLS ASSESSMENTS COMMONLY REQUIRE — Physical examination

Consent/exposure/positioning, correct sequence and technique, patient comfort, recognition of abnormal findings and summary.

Communication

Empathy, plain language, teach-back, shared decisions, interpreter use, difficult conversations and professional boundaries.

Presentation/documentation

Accurate concise oral presentation; problem-oriented note; separate facts from interpretation; acknowledge uncertainty.

Safety/teamwork

Hand hygiene, identity checks, escalation, SBAR/closed-loop communication, medication/allergy awareness and infection prevention.

D. 7-DAY OSCE / CLINICAL-SKILLS REVIEW CYCLE — Days 1-2

Rehearse opening, agenda, HPI and complete/focused history aloud; practice transitions until they sound natural rather than scripted.

D. 7-DAY OSCE / CLINICAL-SKILLS REVIEW CYCLE — Days 3-4

Physical-exam stations. Use a checklist once, then repeat from memory and verbalize what each maneuver is assessing.

Day 5

Documentation + oral presentation. Present the same case in 7-10 min, 3-5 min and 60-second formats; compare what was safely omitted.

Day 6

Timed mixed OSCE circuit: history, exam, counseling/difficult conversation, handoff and note. Use faculty/peer checklist feedback.

Day 7

Targeted repair only. Repeat failed behaviors on a NEW case; do not restart the entire checklist if the weakness is specific.

E. CLINICAL-SKILLS ERROR-LOG TAXONOMY

  • Coverage/technique gap - omitted a required domain or performed the maneuver with incorrect sequence, positioning or interpretation.
  • Communication gap - checklist complete but rapport, empathy, signposting, plain language or shared understanding failed.
  • Synthesis gap - gathered data correctly but could not summarize, prioritize, localize or formulate an assessment.
  • Professional/safety gap - confidentiality, consent, escalation, identity, hand hygiene or supervision boundary was missed.

SCHOOL-EXAM / OSCE MASTERY GATE

  • Proceed when you can complete unfamiliar standardized-patient stations within time, preserve rapport while using a systematic structure, perform core exam maneuvers safely, and convert findings into a concise presentation/note.
  • Competence requires observed practice and feedback from qualified faculty/supervisors; self-study can prepare technique but cannot certify clinical performance. VIDEO-FIRST | SCHOOL-EXAM + BOARD-EXAM MASTERY
