STEP 3 CCS - CASE-SIMULATION MASTERY
POST-GRADUATE PREVIEW | PAGE 36C - STABILIZE -> MANAGE OVER TIME -> REASSESS ->
DISPOSE
CCS is not merely a diagnosis exercise. The software observes how you manage a patient over simulated time: what you order, what you treat, when you advance time, whether you reassess, whether you move the patient to an appropriate setting, and whether you avoid unnecessary or harmful actions.
G. THE 7-STEP CCS CASE LOOP
| Step | What to do | Common failure |
|---|---|---|
| 1. Stabilize | Identify ABC threats / unstable physiology and begin time-critical management. | Delayed stabilization while pursuing diagnosis. |
| 2. Acquire | Focused history/exam + tests that can change the next decision. | Shotgun testing with no decision value. |
| 3. Interpret | Update the differential as new data return; stop tests that no longer matter. | Anchoring despite contradictory data. |
| 4. Treat | Start indicated therapy and discontinue irrelevant/harmful interventions. | Correct treatment at the wrong time or setting. |
| 5. Monitor | Recheck symptoms, vitals, labs and response; advance simulated time deliberately. | Failing to reassess after treatment. |
| 6. Prevent / counsel | Vaccines, screening, risk reduction, adherence, medication counseling and follow-up. | Treating the acute problem but missing longitudinal care. |
| 7. Disposition | Choose home / office / ED / inpatient / ICU / consult/follow-up based on stability and needs. | Unsafe disposition or failure to escalate. |
H. SOFTWARE-SPECIFIC RULES TO PRACTICE
- Location matters. Office, ED, inpatient unit, ICU and home contexts change what should happen next; surgical and labor/delivery facilities are accessed through the system rather than direct transfer.
- Time is part of the exam. Interval history/physical requests and advancing the clock affect when results and treatment effects appear.
- Actions have consequences. Timing/sequencing of indicated actions and potentially harmful actions contribute to evaluation.
- Practice mechanics before content. USMLE explicitly warns that examinees unfamiliar with CCS software mechanics may be disadvantaged.
H. SOFTWARE-SPECIFIC RULES TO PRACTICE — I. CASE-SIMULATION ERROR LOG
- Stabilization error - dangerous physiology recognized late.
- Sequencing error - right action, wrong order or wrong time.
- Monitoring error - treatment given but response not checked.
- Location/disposition error - patient managed in the wrong setting or escalation omitted.
- Preventive/follow-up error - acute care completed but counseling, surveillance or follow-up omitted.
- Software fluency error - lost time because orders, navigation or clock advancement were unfamiliar.
H. SOFTWARE-SPECIFIC RULES TO PRACTICE — CCS MASTERY GATE
Complete official practice cases until the interface disappears from your attention. You should be able to state why each order is being placed, what result you expect, what will change management, when to reassess, and what makes the final disposition safe.
