STRUCTURAL DIAGNOSIS + TECHNIQUE / SAFETY RETRIEVAL
YEAR 2 | PAGE 58C - ERROR TAXONOMY, DIAGNOSTIC CHAIN + SUPERVISED SKILLS BOUNDARY
F. STRUCTURAL-DIAGNOSIS CLOSED-BOOK CHAIN
| Step | Retrieval target |
|---|---|
| 1. History + red flags 2. Observation + palpation | Trauma, neurologic deficit, fracture/infection/malignancy/vascular concern, instability and other reasons manual treatment may be inappropriate. Landmarks, symmetry, TART/tissue texture and tenderness; distinguish reliable findings from uncertain palpation. |
| 3. Regional + segmental motion 4. Name dysfunction | Active/passive ROM and school-taught motion testing; identify restriction before assigning a label. Region + diagnosis + ease/barrier relationship using the school-taught model. |
| 5. Select technique family 6. Safety + consent | Direct vs indirect; active vs passive; treatment goal and patient context. Contraindications/precautions, explanation, consent, draping/comfort and stop criteria. |
F. STRUCTURAL-DIAGNOSIS CLOSED-BOOK CHAIN — G. OMM ERROR-LOG TAXONOMY -> TARGETED REPAIR
7. Reassess + document Repeat relevant finding and document response rather than assuming correction.
F. STRUCTURAL-DIAGNOSIS CLOSED-BOOK CHAIN — G. OMM ERROR-LOG TAXONOMY -> TARGETED REPAIR
| Error | Likely failure | Repair |
|---|---|---|
| Landmark / palpation Biomechanics | Unreliable hand placement or tissue interpretation. Ease/barrier, Fryette, rib, pelvis or sacrum logic reversed. | Return to supervised regional exam; compare findings with faculty/instructor expectations. Redraw the mechanics; verbalize diagnosis -> ease -> barrier before setup. |
| Setup Safety | Diagnosis correct but patient/operator position or activating force wrong. Contraindication, consent or red flag missed. | Rehearse under supervision; do not reinforce the wrong motor pattern alone. Build contraindication/precaution retrieval into every technique family. |
| Integration | OMM fact recalled but detached from presentation/anatomy/standard workup. | Solve new clinical vignettes and explain when OMT is adjunctive vs inappropriate. |
F. STRUCTURAL-DIAGNOSIS CLOSED-BOOK CHAIN — 7. Ease/barrier logic prevents reversed diagnosis and setup DEEP EXPLANATION Biomechanics questions often become easier when you first identify where the segment moves freely and where motion is restricted. That logic anchors the diagnosis and any direct/indirect setup. WHY STUDENTS MISS IT Learners memorize diagnostic labels or hand positions separately from the motion that defines them. EASIEST CLUE When confused, stop naming the technique and state the ease and restrictive barrier first. 8. Viscerosomatic and Chapman associations are adjunct clues, not substitutes for diagnosis DEEP EXPLANATION Autonomic and Chapman maps can support an osteopathic interpretation, but they do not replace history, physical examination, standard diagnostic testing or urgent medical evaluation. WHY STUDENTS MISS IT Highly memorable association maps can become over-weighted in board review. EASIEST CLUE If the stem contains a dangerous conventional diagnosis, address that first; use OPP relationships only in the appropriate sequence. 9. Manual-skill mastery requires supervised feedback DEEP EXPLANATION Palpation, positioning, force, tissue response and patient communication are psychomotor behaviors. Videos can clarify sequence, but instructor feedback is needed to correct motor errors and verify safe
execution.
