COMLEX-USA LEVEL 2-CE - DIMENSION 1 + CURRENT FORMAT
YEAR 3 | PAGE 68A - COMPETENCY DOMAINS -> CLINICAL DECISION TASKS -> FORMAT -> BLUEPRINT VERSION CONTROL
BLUEPRINT RULE: LEVEL 2-CE IS NOT A BODY-SYSTEM EXAM WITH OMM ADDED ON TOP. EVERY ITEM LIVES IN A TWO-DIMENSION BLUEPRINT: A PHYSICIAN COMPETENCY IS TESTED THROUGH A CLINICAL PRESENTATION. STUDY THE CLINICAL CORE, BUT TRACK WHICH COMPETENCY THE QUESTION ACTUALLY REQUIRES. A. DIMENSION 1 - CURRENT LEVEL 2-CE MINIMUMS
| Competency domain | Minimum | What the learner must be able to do |
|---|---|---|
| 1. Osteopathic Principles, Practice & Manipulative Treatment | 10% | Integrate osteopathic principles, structural findings, OMT indications/contraindications and osteopathic reasoning into clinical care. |
| 2. Osteopathic Patient Care & Procedural Skills | 30% | Acquire/interpret clinical data; select diagnostic and management steps; perform or understand appropriate patient-care/procedural actions. |
| 3. Application of Knowledge for Osteopathic Medical Practice 3.1 Foundational Biomedical Sciences Knowledge Base | 26% 25% nested | Apply clinical and foundational science to diagnosis, mechanisms, risk, treatment and prevention. Foundational science remains embedded within clinical reasoning; treat this as a nested blueprint element, not an additional 25% to add to the total. |
| 4. Practice-Based Learning & Improvement 5. Interpersonal & Communication Skills | 7% 5% | Use evidence, quality data, self-evaluation and improvement principles to improve care. Choose communication behaviors appropriate to patients, families, teams and difficult clinical situations. |
| 6. Professionalism 7. Systems-Based Practice | 7% 5% | Apply ethics, boundaries, consent, confidentiality, duty and professional responsibility. Navigate safety, teams, transitions, resources, population/system constraints and care delivery. |
B. CURRENT DELIVERY MODEL + BLUEPRINT VERSION
| Anchor | Current 2026/27 specification | Study implication |
|---|---|---|
| Exam structure Question media | 320 single-best-answer MCQs; 8 sections x 40 questions; two timed 4-hour sessions in one day. Some items may include images, video or audio. | Rehearse 40-question section pacing, repeated section resets and full-day stamina. Do not restrict preparation to text-only vignettes; review official practice resources and audiovisual exhibits. |
| Blueprint version | For Level 2-CE testing on or before Apr 22, 2028, NBOME directs candidates to the revised Feb 2025 blueprint. Blueprint 2.0 applies to June 2028 or later. | For a 2026/27 test date, use the Feb 2025 blueprint; re-check NBOME if the test date changes. |
B. CURRENT DELIVERY MODEL + BLUEPRINT VERSION — 1. The Level 2-CE blueprint is a two-dimensional matrix, not two independent study lists DEEP EXPLANATION Dimension 1 describes what physician competency is being tested; Dimension 2 describes the clinical presentation through which it is tested. A chest-pain item may test
knowledge, patient-care sequencing, communication, professionalism, systems practice or osteopathic integration depending on the task.
B. CURRENT DELIVERY MODEL + BLUEPRINT VERSION — WHY STUDENTS MISS IT Students often study one list as competencies and another as body systems, then fail to connect them during questions. That creates knowledge without task recognition. EASIEST CLUE For every miss ask two questions: What presentation was this? What competency failed? 2. Blueprint percentages are minimum test specifications - not a literal hour-by-hour study budget DEEP EXPLANATION The percentages define examination representation, but study time should also reflect the learner's own error recurrence, clerkship decay and readiness evidence. A 5% domain that
is repeatedly missed may deserve more than 5% of remediation time.
WHY STUDENTS MISS IT Percentages feel objective, so learners may convert them directly into a calendar even when personal weaknesses are highly uneven. EASIEST CLUE Use blueprint weights to protect coverage; use your errors to set intensity. 3. The 30% Patient Care domain is broader than memorizing procedures DEEP EXPLANATION Patient care includes acquiring and interpreting clinical information and choosing appropriate diagnostic, therapeutic, preventive and procedural actions. Many Level 2-CE questions
therefore hinge on the next correct action rather than disease-name recall.
WHY STUDENTS MISS IT The word "procedural" can make students overfocus on technique trivia while undertraining clinical sequencing and management. EASIEST CLUE When a vignette asks what to do next, think Patient Care before pure recall.
