INTERNAL MEDICINE - SHELF / COMAT PROBLEM SOLVING

YEAR 3 | PAGE 29C - QUESTION BANK + ERROR-LOG MASTERY

E. QUESTION-REVIEW ALGORITHM

StepAsk yourself
1. FrameWhat is being asked: diagnosis, next test, treatment, prevention, prognosis, complication, adverse effect or disposition?
2. StabilizeIs there a life-threatening condition that changes the sequence?
3. LocalizeWhat organ system / syndrome best explains the stem?
4. DiscriminateWhich 2-3 clues truly separate the top answers?
5. SequenceWhat is the next step now, not the ultimate definitive step later?
6. ReviewWhy is the right answer right, why is the best distractor wrong, and what rule will prevent the same miss?

E. QUESTION-REVIEW ALGORITHM — F. MEDICINE ERROR TAXONOMY

  • Recognition gap - did not identify the syndrome or classic pattern.
  • Diagnostic-sequence gap - knew the disease but chose a confirmatory test before an indicated screening/initial test, or vice versa.
  • Management-sequence gap - chose a correct therapy at the wrong time or after a missed stabilization step.
  • Medication/safety gap - contraindication, adverse effect, renal/hepatic dosing, interaction or monitoring requirement.
  • Disposition/prevention gap - missed admission/ICU need, follow-up, screening, vaccination or secondary prevention.
  • Reasoning/timing gap - anchoring, ignoring a decisive clue, overtesting or running out of time.

G. 7-DAY INTERNAL MEDICINE EXAM FINISH

DayExecution
7-6Map weak systems from Qbank analytics + clerkship feedback; close the two largest mechanism gaps.
5-4Timed medicine blocks; review every incorrect, guessed and low-confidence correct; repair only identified gaps.
3Take NBME Medicine CSMS or COMAT-SE if scheduled/available; use domain feedback, not one question, to redirect study.
2Mixed timed work + high-yield algorithms: ACS, dyspnea, sepsis, AKI/electrolytes, GI bleed/cirrhosis, diabetes emergencies, anemia.
1Light retrieval, error sheets, medications, prevention; stop heavy new learning early and protect sleep.
