TRANSITIONS OF CARE + RESIDENCY READINESS
YEAR 4 | PAGE 34D - TRANSITIONS OF CARE ARE CLINICAL PROCEDURES
THE SAFETY IDEA Many Year 4 errors are not rare-disease errors; they are failures to notice deterioration, prioritize a page, follow a result, reconcile medications, communicate uncertainty or transfer responsibility. Treat every handoff and discharge as a high-risk clinical procedure.
J. CROSS-COVER / AFTER-HOURS RESPONSE
| When paged | First questions | Safe output |
|---|---|---|
| “Patient looks worse” | What changed? Current vitals/oxygen/mental status? What is the baseline? What has already been done? | Bedside review when indicated, instability screen, focused action, senior escalation and reassessment. |
| Abnormal lab / result | Is it real/critical? Trend? Symptoms? Medication/renal/hepatic context? Does it require action tonight? | Interpret result in context, treat/confirm if needed, communicate, document and hand off unresolved follow-up. |
| Pending task / consult | Why is it pending? What decision depends on it? What is the deadline? | Close the loop or explicitly transfer ownership with a contingency and time trigger. |
J. CROSS-COVER / AFTER-HOURS RESPONSE — K. HANDOFF + DISCHARGE CHECK
- Handoff: illness severity -> summary -> active problems -> action list -> pending tests -> contingencies -> escalation limits -> receiver questions.
- Discharge + closed loop: stability -> medication reconciliation -> pending tests/follow-up owner -> return precautions -> barriers -> patient understanding; responsibility and the next action must be explicit.

L. AAMC RESIDENCY-READINESS CROSSWALK — M. GRADUATION-TO-RESIDENCY MASTERY GATE
- Can run a supervised patient day from pre-rounding through handoff without losing pending results or safety tasks.
- Recognizes deterioration, calls for help early, performs an organized initial assessment and reassesses after interventions.
- Can interpret common CXR/ECG/basic imaging patterns and prepare safely for common procedures within the granted supervision level.
- Can give a concise handoff, manage common cross-cover tasks, reconcile medications, build a safe discharge plan and communicate personal limits.
