Medical School MD + DO Mastery

A 4-year, board-aligned study system for medical school and licensing exams.

Walk the full Year 1–4 curriculum — foundational sciences, organ-system blocks, clinical clerkships and advanced practice — each with mastery checklists and linked learning resources, plus a dedicated review system for school exams, Shelf/COMAT, USMLE Step 1–3 and COMLEX-USA Levels 1–3.

INTERNAL MEDICINE - GI / RENAL / ENDOCRINE

YEAR 3 | PAGE 61B - VOLUME / ELECTROLYTES -> BLEED / CIRRHOSIS -> DIABETES / THYROID / ADRENAL

C. HIGH-VALUE SYSTEM / SYNDROME MAP

DomainCore presentationsManagement sequence to retrieve
Renal / volumeAKI/CKD, oliguria, volume depletion/overload, nephrotoxic injury, obstruction.Assess severity/volume + medications -> urine/basic labs/imaging as indicated -> reverse cause -> monitor K/acid-base/volume -> dialysis triggers when present.
Electrolyte / acid-base GI bleedNa/K disorders, metabolic acidosis/alkalosis, mixed disorders. Upper vs lower source, anemia, anticoagulation, hemodynamic compromise.Confirm pattern -> identify dangerous manifestations -> correct at safe rate -> treat cause -> recheck trajectory. Stabilize -> access/labs/transfusion strategy -> medication reversal/support when appropriate -> endoscopic/radiologic localization -> disposition.
Liver / portal HTN EndocrineCirrhosis, ascites, encephalopathy, variceal bleeding, infection, hepatorenal physiology. DKA/HHS, thyroid disease, adrenal crisis, calcium disorders, inpatient glucose.Identify precipitant/complication -> treat immediate threat -> diagnose complication -> prevent recurrence / address chronic disease. Recognize physiologic deficit/excess -> stabilize fluids/electrolytes -> hormone/drug therapy in correct sequence -> monitor transition/complications.

C. HIGH-VALUE SYSTEM / SYNDROME MAP — 4. AKI is a mechanism + trajectory problem, not just a creatinine number DEEP EXPLANATION Internal-medicine questions ask why kidney function changed and what must be reversed now. Volume status, hemodynamics, nephrotoxins, obstruction, urinalysis/sediment and the time course separate

prerenal/hemodynamic, intrinsic and postrenal patterns. Severity is defined by consequences as well as the creatinine.

C. HIGH-VALUE SYSTEM / SYNDROME MAP — WHY STUDENTS MISS IT Students see a high creatinine and jump directly to a named renal disease or an invasive test. EASIEST CLUE Start with volume / perfusion -> medications/toxins -> obstruction -> urine pattern; then ask whether K, acid-base, volume or uremia creates an urgent indication for escalation. 5. GI bleed and cirrhosis questions separate resuscitation from source diagnosis DEEP EXPLANATION A patient with bleeding or decompensated liver disease may need stabilization and complication-specific treatment before definitive source localization. Cirrhosis questions also test recognition of ascites infection,

encephalopathy, variceal disease, renal dysfunction and recurrent-prevention logic.

WHY STUDENTS MISS IT The memorable procedure - endoscopy, paracentesis or imaging - distracts from the immediate physiology and treatment sequence. EASIEST CLUE Ask is the immediate problem blood loss, infection, pressure/portal complication, mental status or renal perfusion? Fix the dangerous physiology while pursuing the source. 6. Endocrine emergencies are ordered algorithms built from physiology DEEP EXPLANATION DKA/HHS, adrenal crisis and severe thyroid disease are not solved by naming the disorder alone. Fluids, electrolytes, hormone replacement or insulin may have a required order because one intervention changes the

safety of the next. The same principle applies to transition and monitoring.
