YEAR 3 - NBME / COMAT READINESS LADDER
YEAR 3 | PAGE 33C - OFFICIAL SELF-ASSESSMENTS + 14-DAY / 7-DAY / 48-HOUR FINISH
Use official-style assessments as diagnostic instruments, not as trophies. One score is noisy; repeated domain patterns, time use, question-level reasoning and the ability to repair errors are more actionable. Your school's grading policy and exam instructions remain the source of truth.
I. CURRENT OFFICIAL ASSESSMENT MAP
| Tool | Current official signal | Best use |
|---|---|---|
| NBME CSMS | Clinical Science Mastery Series covers Clinical Neurology, Emergency Medicine, Family Medicine, Medicine, OB/GYN, Pediatrics, Psychiatry and Surgery. Each form has 50 MCQs. Standard-paced mode is 75 minutes; self-paced is 5 hours. | Late-rotation readiness check + content-area feedback + approximate subject-exam score translation. Use errors to redirect the final review. |
| NBME Subject Exam | Clinical Science Subject Exams are school-administered end-of-clerkship assessments. Item count and timing vary by exam; follow the current NBME timing chart and your school instructions. | Train with timed blocks and the current interface, but do not assume every shelf has the same item count or timing. |
| COMAT Clinical | Each COMAT Clinical Subject Exam has 120 items in 2.5 hours. OPP/OMM is integrated into the discipline blueprint. | DO end-of-rotation assessment: prepare the clinical discipline first, then integrate osteopathic framing where the blueprint makes it relevant. |
| COMAT-SE | Formative self-evaluation aligned to COMAT Clinical blueprints. Current forms contain 60 questions with immediate answers/rationales. Available for FM, IM, Pediatrics, OPP, EM, Surgery, OB/GYN and Psychiatry. | Use for learning and gap identification before the COMAT. Neurology does not currently have a standalone COMAT Clinical / COMAT-SE route. |
J. THE FINAL 14 DAYS - ACADEMY READINESS SEQUENCE
| Window | Execution |
|---|---|
| 14-10 days | Take the first formal readiness check if it will leave time to act on the result. Map misses by domain + error type. Repair only the largest 2-3 gaps; keep daily rotation questions going. |
| 9-7 days | Run timed blocks in the exam's dominant reasoning style: diagnosis / next test / management / prevention / disposition. Re-test repaired topics with NEW items. |
| 7-4 days | Use a second official-style check if available and appropriate. Compare patterns across assessments rather than obsessing over a single unusual question. Tighten pacing. |
| 3-2 days | Mixed timed work + highest-yield algorithms + medication/safety + prevention/disposition + error-sheet retrieval. Do not restart entire textbooks or video libraries. |
| Final 48 hours | Light retrieval, logistics, current exam software/tutorial familiarity, sleep and nutrition. Stop heavy new learning early enough to preserve cognition and endurance. |
K. READINESS GATE - SIX SIGNALS
- Coverage: all blueprint domains have been sampled; no major content area was accidentally skipped because you never saw it clinically.
- Trend: timed Qbank and official-style performance is stable or improving across several sets, not dependent on one score.
- Error stability: recurring knowledge and sequence errors are shrinking; the same mistake is not appearing every week.
- Timing: you can finish appropriately sized timed blocks without a late-block collapse in accuracy.
- Reasoning: you can explain why the next step comes before the definitive step and why the closest distractor is wrong.
- Clinical safety: you identify instability, medication contraindications, escalation needs and disposition red flags reliably.
K. READINESS GATE - SIX SIGNALS — L. INTERFACE / TEST-DAY PRACTICE
NBME completed the current CSMS interface rollout by January 2026 and updated its comprehensive self-assessment interface in June 2026. Use the official sample assessment/tutorial to practice navigation, lab search, highlighting, image controls and notes. For the actual shelf/COMAT, follow your school's secure-testing instructions and accommodations.
