SURGERY - NBME SHELF / COMAT TRANSFER
YEAR 3 | PAGE 29H - OFFICIAL BLUEPRINT + QUESTION-SEQUENCING MASTERY
H. WHAT THE OFFICIAL EXAMS ARE ASKING YOU TO DO
| Lane | Current official signal | How to prepare |
|---|---|---|
| NBME Surgery | Clinical Science Mastery Series includes Surgery. Each self-assessment form has 50 MCQs; standard-paced forms allow 75 minutes and provide answer explanations plus approximate Subject Exam score translation. | Use one or more timed forms late in the clerkship. Review why the correct next step beats the most attractive distractor, then target recurring content categories. |
| COMAT Surgery blueprint | Largest current content ranges include GI/Nutrition 18-24%, Perioperative Care 16-20%, Hepato/Biliary 14-20% and Trauma 14-18%; hernia, breast, endocrine and integument/MSK remain active. | Do not study only "operations." Keep perioperative care, nutrition, trauma, hepatobiliary disease and common general-surgery presentations active. |
| COMAT format / tasks | COMAT Clinical is 120 items in 2.5 hours with OPP/OMM integrated. Surgery physician-task ranges: H&P;/Diagnosis 25-35%, Diagnostic Technologies 25-35%, Management 25-35%, Mechanisms 10-15%. | Practice sustained pacing and management sequencing. DO learners should integrate appropriate OPP/OMM without delaying urgent stabilization or source control. |
I. SURGERY QUESTION-REVIEW ALGORITHM
- 1. Stabilize: Is there shock, airway/breathing failure, uncontrolled bleeding, peritonitis, ischemia or sepsis that changes the sequence?
- 2. Name the syndrome: obstruction, perforation, infection/inflammation, ischemia, hemorrhage, mass/endocrine, breast/vascular or postoperative complication.
- 3. Decide the intervention class: observe/support, image, antibiotics, anticoagulate, drain/decompress, endoscope/IR, operate or transfer/escalate.
- 4. Sequence: answer the next step now - not the eventual definitive procedure if resuscitation or diagnostic discrimination comes first.
- 5. Debrief: why is the best distractor wrong? Wrong diagnosis, wrong timing, wrong stability assumption, or correct treatment at the wrong stage?
J. 7-DAY SURGERY EXAM FINISH
| Day | Execution |
|---|---|
| 7-6 | Map misses into four buckets: perioperative/post-op, trauma, abdomen/GI-hepatobiliary, and breast/hernia/endocrine/vascular. Repair the two largest gaps. |
| 5-4 | Timed Surgery shelf/Step 2 blocks. Review every incorrect, guessed and low-confidence correct; write one sequencing rule per miss. |
| 3 | Take NBME Surgery CSMS or COMAT-SE if scheduled/available; use domain patterns to redirect the last 48 hours. |
| 2 | Mixed acute abdomen + trauma + postoperative algorithms; drill fluids/electrolytes, nutrition and common operative indications/complications. |
| 1 | Light retrieval and error sheet only. Rehearse "unstable first" and next-step algorithms; stop heavy new learning early and protect sleep. |
J. 7-DAY SURGERY EXAM FINISH — K. CLERKSHIP MASTERY GATE
- Can explain the indication, anatomy, expected course and major complications for a patient's operation.
- Can recognize postoperative deterioration and choose the first safe diagnostic/management step.
- Can move from trauma or acute-abdomen physiology to the correct imaging / resuscitation / source-control sequence.
- Can complete timed mixed Surgery questions with stable accuracy and a shrinking recurrence-error pattern. CLINICAL BOUNDARY This Academy supports medical education and supervised clerkship performance. Real trauma, perioperative and operative decisions must follow your level of training, supervising clinicians, institutional protocols and current specialty guidance. Official / question-bank links
