COMLEX LEVEL 3 + FIRST 90 DAYS OF RESIDENCY

DO POST-GRADUATE PREVIEW | PAGE 36D - MCQ + CDM -> OSTEOPATHIC GENERALIST PRACTICE

Level 3 assesses the competencies expected for unsupervised osteopathic medical practice. Because the exam changes during the 2026-27 cycle, study plans must match the actual testing date rather than relying on older schedules or question counts.

J. LEVEL 3 FORMAT - CURRENT VS JANUARY 2027

Testing dateFormatQuestion / case structure
Through Dec 19, 2026Two-day computer-based exam within the current administration model.420 single-best-answer MCQs + 26 CDM cases.
On/after Jan 13, 2027One-day, 8-hour computer-based exam; four 2-hour sessions.240 single-best-answer MCQs + 16 CDM cases; blueprint remains unchanged.

K. CDM CASE LOGIC

  • Cases contain 2-4 questions focused on data acquisition, interpretation/diagnosis, treatment, counseling and follow-up.
  • Response types include extended multiple choice and short answer. Some extended-MCQ prompts may require more than one correct option; obey response limits.
  • Progression is consequential. Once an earlier case part is submitted, you cannot return and change it. Commit only after checking the task and response limit.
  • Osteopathic framing stays active. OPP/OMM, osteopathic patient care, communication, professionalism and systems-based practice remain part of the Level 3 blueprint.

L. READINESS TOOLS

ToolRole / limitation
Official NBOME CDM tutorial + sample casesMandatory format familiarity: response limits, short-answer mechanics, irreversible progression and case navigation.
COMSAE Phase 3200 questions in four 50-question sections; aligned to Level 3 content, but it does NOT include CDM cases or avatars. Use with other readiness evidence.

M. FIRST-90-DAY RESIDENCY SCAFFOLD - ACADEMY TRAINING MODEL

WindowPrimary target
Days 1-30Learn local systems, escalation pathways, documentation/order workflows and patient-safety norms. Prioritize reliability over speed.
Days 31-60Improve anticipation: pending results, likely overnight problems, discharge barriers, medication monitoring and contingency planning.
Days 61-90Increase efficiency without skipping safety checks; use feedback/error trends to refine presentations, cross-cover and independent study.

M. FIRST-90-DAY RESIDENCY SCAFFOLD - ACADEMY TRAINING MODEL — N. TRANSITION-TO-RESIDENCY MASTERY GATE

  • Can recognize deterioration, initiate appropriate supervised stabilization and escalate with a concise problem representation and explicit request.
  • Can safely manage the information loop: orders/tasks -> results -> interpretation -> reassessment -> communication -> handoff.
  • Can approach Step 3 CCS / Level 3 CDM as longitudinal management problems rather than one-shot diagnosis questions.
  • Can use exam study without allowing it to displace orientation, patient safety, sleep/recovery and supervised clinical learning during early residency. NBOME Level 3 | 2026-27 Level 3 format | Official CDM tutorial | COMSAE Phase 3

M. FIRST-90-DAY RESIDENCY SCAFFOLD - ACADEMY TRAINING MODEL — CLINICAL / EXAM BOUNDARY

This Academy supports education, supervised transition-to-residency practice and exam preparation. Real patient-care decisions, orders, procedures and escalation must follow your scope, supervising clinicians, institutional policy and current clinical guidance. Official USMLE/NBOME materials are the source of truth for exam eligibility, format and test-day policy. 2026/27 transition-to-residency expanded edition - verify official exam updates before testing
