Medical School MD + DO Mastery

A 4-year, board-aligned study system for medical school and licensing exams.

Walk the full Year 1–4 curriculum — foundational sciences, organ-system blocks, clinical clerkships and advanced practice — each with mastery checklists and linked learning resources, plus a dedicated review system for school exams, Shelf/COMAT, USMLE Step 1–3 and COMLEX-USA Levels 1–3.

HEME / ONC - INFECTIOUS DISEASE INTEGRATED TRANSFER

YEAR 2 | PAGE 49D - CBC / SMEAR / COAGULATION / MALIGNANCY / HOST DEFENSE / ANTIMICROBIALS + 10 CASES

INTEGRATION RULE: BLOOD, CANCER AND INFECTION INTERACT THROUGH MARROW FUNCTION, IMMUNE COMPETENCE, VASCULAR / COAGULATION PHYSIOLOGY AND TREATMENT TOXICITY. IDENTIFY THE DOMINANT THREAT FIRST, THEN CONNECT THE LAB PATTERN, HOST STATE, ORGANISM OR DRUG MECHANISM. J. CROSS-SYSTEM INTEGRATION MAP

LANEMUST RETRIEVE WITHOUT NOTESHIGH-DISCRIMINATION TRANSFER
Sepsis -> coagulationSevere infection can trigger endothelial injury, cytokine activation and consumptive coagulopathy.Use platelet/PT/aPTT/fibrinogen/D-dimer plus clinical instability to separate DIC from isolated thrombocytopenia or factor deficiency.
Cancer -> infectionMarrow failure, neutropenia, mucosal injury, devices and immunosuppression change likely pathogens and urgency.Fever in an immunocompromised host is not the same pretest probability as fever in an immunocompetent host.
Cancer -> metabolic emergencyHigh tumor burden and treatment can cause TLS, hypercalcemia, leukostasis or compression syndromes.Recognize the emergency before spending time on complete histologic classification.
Antimicrobials -> bloodSome drugs cause marrow suppression, hemolysis, platelet dysfunction or interactions with anticoagulants.Predict toxicity from drug mechanism and patient risk instead of memorizing a detached adverse-effect list.
Transfusion -> infection/inflammationTransfusion reactions can mimic sepsis or cardiopulmonary deterioration; timing and phenotype matter.Use reaction timing, fever/hypotension, hemolysis, pulmonary edema and volume clues to identify the pathway.

K. 10 HIGH-DISCRIMINATION INTEGRATED CASES

#CASEANSWER LOGIC
1Microcytic anemia with low ferritin and high TIBC.Iron deficiency: first interpret iron-store/transport pattern, then investigate the source of iron loss rather than stopping at the MCV label.
2Anemia + schistocytes + thrombocytopenia + prolonged PT/aPTT + low fibrinogen in a septic patient.Consumptive coagulopathy/DIC pattern: infection plus global coagulation-factor/platelet consumption; distinguish from TTP/HUS where routine coagulation tests are often not globally prolonged.
3Mucosal bleeding, normal PT, prolonged aPTT that corrects on mixing.Factor deficiency pattern; correction with mixing argues against a strong inhibitor. Clinical bleeding phenotype and vWF/factor context narrow further.
4Older patient, lymphocytosis, smudge-cell-type pattern, mature B-cell process.Use age + chronic lymphoid lineage + morphology/flow context to favor CLL over acute leukemia.
5Bone pain, anemia, renal dysfunction, hypercalcemia, monoclonal protein.Plasma-cell disorder/myeloma pattern. The organ-damage cluster is more discriminating than the M-spike alone.
6 7New chemotherapy, rising K/phosphate/uric acid, falling calcium, AKI. Neutropenic patient with fever and no clear source.Tumor lysis syndrome: treatment/high burden -> rapid cell breakdown -> metabolic emergency. Treat as an urgent host-risk problem: broad empiric coverage must not wait for a perfect localization; cultures/source assessment follow without delaying care.
8Meningitis syndrome in an older or immunocompromised host.Start with syndrome + host to expand the empiric pathogen set; choose CSF/imaging sequence and antimicrobial coverage appropriate to risk.
9Positive blood cultures with a prosthetic device and persistent bacteremia.Device/biofilm context changes pathogen probability and may require source-control reasoning in addition to antibiotics.
10Culture identifies a susceptible narrow-spectrum option after broad empiric therapy.Definitive phase: de-escalate to the narrowest effective therapy that reaches the site and fits allergy/organ-function constraints.

L. OFFICIAL PRACTICE + READINESS STACK — M. THREE-DOMAIN MASTERY GATE

Ready to move on when you can: (1) classify anemia/hemolysis and coagulation problems from CBC, retic, smear and labs; (2) identify major hematologic malignancies from lineage, maturity, age, flow/cytogenetic and organ patterns; (3) distinguish grade, stage, treatment toxicity and oncologic emergencies; (4) move from infection syndrome + host -> pathogen set -> specimen/test -> empiric -> narrowed therapy -> resistance/prevention; and (5) maintain stable performance on new mixed questions with recurring errors shrinking. This is an educational review framework; real patient-care decisions require supervising clinicians, institutional policy and current clinical guidance. 2026/27 systems-review expanded edition - align with school objectives and verify official exam updates before testing
